- Student's name
- Grade
- School
- Any additional pertinent information about the student's situation
- Your contact information and relation to the student

If I'm unable to take your call, I will get back to you as soon as possible, usually within 12-72 hours.

Pre-intake phone or e-mail consultation is provided free of charge.

Please pay by cash or check at each session. Regular students are eligible for monthly billing, if desired.

There is no commitment for any number of sessions, and pre-payment is never required.
